Usually a worn mortise lock or a keeper plate that has drifted out of alignment as the panel settled. This one is a security issue, so it is worth handling the same week.
A single pane can usually be cut and replaced without touching the rest of the door. Haze between two panes means the seal failed and the insulating gas has already escaped.
